Emergency and Primary Care at a Public Facility, by Statute

Belize wrote the health entitlement into an Act rather than leaving it to a regional announcement. Section 13 of the Caribbean Community (Free Movement and Contingent Rights) Act 2026 entitles a Community national and their family members to emergency and primary health care at a public hospital, clinic or health centre. It is the entitlement most of the region states through its foreign ministry and does not legislate.

Belize's National Health Insurance Scheme is open to foreigners on the face of the statute: section 72 of the Social Security Act makes Belizean nationals and all persons legally resident in Belize eligible to enrol, and enrolment is a matter of producing documents establishing eligibility rather than of a contribution record. Separately, anyone in insurable employment between fourteen and sixty-five is insured under the Social Security Act itself, regardless of nationality - the test is the employment, not the passport. The third layer is the newest and the narrowest: a national of Barbados, Dominica or Saint Vincent and the Grenadines admitted under the 2026 free movement Act has a statutory entitlement to emergency and primary healthcare at public facilities without discrimination on the ground of nationality.

Requirements

  • For the National Health Insurance Scheme: Belizean nationality or legal residence in Belize, and enrolment by producing documentation establishing eligibility
  • For social security cover: employment in an insurable employment, being over fourteen and under sixty-five
  • For a Temporary Employment Permit holder: registration with the Social Security Board, which the Immigration Department makes the last step of the permit process
  • For a permanent residence applicant: a medical examination in Belize, with HIV, VDRL and TB tests
  • For a Qualified Retired Person: a complete medical examination including an HIV test, no older than three months

In detail

Legal residence is the key, not contributions

Most of the region ties public healthcare to a contribution record: you pay in, through employment or voluntarily, and then you are covered. Belize's National Health Insurance Scheme is drawn differently. Section 72 makes Belizean nationals and all persons legally resident in Belize eligible to enrol, and section 72(2) makes enrolment a matter of producing documentation establishing that eligibility. Contributions are dealt with separately in section 71, which empowers the Board to require them from the employed, employers, the self-employed and retired people on a pension - and provides that the Government pays for indigents and for retirees whose only income is a pension or annuity not exceeding BZ$5,000 a year.

Working makes you insured, whatever your passport

Section 3(1) of the Social Security Act insures every person over fourteen and under sixty-five who is employed in insurable employment. There is no nationality condition and no immigration condition. In practice the Immigration Department builds registration into the work permit: the last step of the Temporary Employment Permit process is to visit the Social Security Office and apply for a card, and a Social Security letter is one of the documents Immigration asks for before issuing the permit. A permanent residence application in turn asks for the record of contributions made on your behalf as evidence of financial stability, so the three systems reference each other.

What the free movement Act adds

The Caribbean Community (Free Movement and Contingent Rights) Act 2026 gives a national of Barbados, Dominica or Saint Vincent and the Grenadines, and their family members, a statutory entitlement to emergency and primary health care at a public hospital, clinic or health centre without discrimination on nationality. The Immigration Department's own advice to people arriving under that regime is to register with the Social Security Board to access the applicable benefits. Note what the section does and does not cover: emergency and primary care at public facilities, not the full range of NHI services.

Every Chapter of the Substantive Laws of Belize that you can download today is the Revised Edition 2020, and each one says on its face that it shows the law as at 31 December 2020. Amendments made since are published separately, as numbered Acts and statutory instruments, and they are not worked into the Chapter. The consequence is practical and it bites hardest on the two subjects people read about most: the Chapter on the retirement programme still prints an age of forty-five that an Act of 2023 reduced to forty, and the Chapter on CARICOM skilled persons still lists thirteen qualifying states when an Act of 2026 removed three of them. Reading the Chapter and stopping there produces a confidently wrong answer. The Chapter plus every amending Act since 2021 is the law.

Practical notes

The National Health Insurance Scheme was rolled out geographically rather than nationally, and nothing in the Act tells you which districts are covered. We have not read a Ministry of Health source stating the current geography, so this record states eligibility and not availability.

A Qualified Retired Person is deemed a non-resident for the purposes of the Immigration Act and the Belizean Nationality Act, but section 4(5) of the Retired Persons (Incentives) Act does not list the Social Security Act. Whether a QRP is "legally resident in Belize" for section 72 is not something we found answered in any instrument, and we do not state it either way.

Both the permanent residence application and the Qualified Retired Persons application require an HIV test, which is unusual among the countries covered here and is stated by the Immigration Department and by the gazetted QRP application form respectively.
