Costa Rica runs a single public health system through the Caja Costarricense de Seguro Social, and for residents joining it is not a choice. The migration law makes affiliation a condition of renewing the residency card, for permanent and temporary residents alike, and it requires the cover to have run without interruption from the moment residency was granted - so a gap in contributions is a gap in the migration file. The directorate asks for proof of affiliation at every renewal appointment, naming the voluntary scheme and the employer's payroll slip as the usual forms. Non-residents are in a different position entirely: remote workers must carry private medical cover of at least US$50000 for the whole of the authorised stay, and nothing in the visitor rules requires any health cover at all.

Requirements

  • Affiliate with the CCSS as soon as residency is granted - as a salaried worker through an employer, as a self-employed contributor, or as a voluntary contributor
  • Keep contributions unbroken from the grant of residency until the card is renewed
  • Bring proof of affiliation to every renewal appointment - the voluntary-insurance receipt, the employer's payroll slip, or equivalent
  • For executives, managers, representatives and technical staff, show affiliation as a condition of renewing the residency card under article 83

Residency and the Caja are the same obligation

In most countries, joining the health system is a separate administrative act with its own consequences. In Costa Rica it is a condition of keeping the residency itself. The law requires proof of affiliation at renewal, running without interruption from the day residency was granted. A retiree who lets contributions lapse for six months has not only lost cover; they have a documentary problem at the next appointment that the law does not provide an obvious way to cure.

Which scheme a resident joins

An employee is enrolled by the employer and shows the payroll slip. The self-employed join as independent contributors. Everyone else - retirees, people living on rents, dependants - joins as a voluntary contributor, which is the route the directorate names first on its renewal checklist. Contributions are assessed on declared income rather than charged as a flat premium, so the cost of residency includes a monthly percentage that none of the income thresholds mentions.

Visitors and remote workers are outside it

Nothing in the entry requirements asks a visitor for health insurance. The one non-resident category that does is the remote worker, and there the requirement is explicit and quantified: medical cover of at least fifty thousand dollars for the whole authorised stay, for each person in the family group, from a locally authorised insurer or an international policy with the same cover. Remote workers do not join the Caja and are not entitled to it.

Costa Rica attaches a running obligation to residency that most countries leave to the health system to enforce. Affiliation to the Caja Costarricense de Seguro Social is compulsory for permanent and temporary residents, it must run without interruption from the day residency was granted, and the proof is demanded at every renewal of the residency card. A lapse in contributions is therefore not only a loss of cover but a defect in the migration file. The rule cuts across every residency category: the retiree with a US$1000 pension, the rentista, the investor who may not work, the spouse of a Costa Rican and the employee of a Costa Rican company are all inside it, and the only exceptions are whatever the executive sets by regulation.

For the purposes of renewing their migration status, and where applicable, permanent residents must show their affiliation to the CCSS schemes, at least from the moment that residency was granted to them and without interruption up to the moment of renewing their foreigner's identity card. Exceptions to this rule shall be established by regulation.

Two words do a lot of work. Ininterrumpida means a lapse is not cured by paying up on the eve of the appointment. Vía reglamento means the exceptions are the executive's to define, so the statute cannot tell you who is outside the duty.

Practical notes

The statute says the exceptions to compulsory affiliation will be set by regulation. We did not find them in the articles of the Reglamento de Extranjería we read, and we are not asserting that none exist.

We looked for the CCSS's own published rules on affiliating foreign residents on its website and did not find a document stating them. What is recorded here about compulsion comes from the migration law and from the migration directorate's renewal checklists, which is the state's own statement of what it will check, but it is not the Caja's statement of what it requires.

Contribution rates and the minimum contribution base change by board resolution and are not reproduced here; any figure quoted for the monthly cost of voluntary affiliation goes stale quickly.

Emergency treatment in a public hospital is provided regardless of affiliation, but the bill follows an uninsured patient. That is a general statement about how the system operates and is not sourced to an instrument here.
