Panama's social security is tied to work, not to residence. Every worker in the country, national or foreign, employed or self-employed, must join the Caja de Seguro Social - and the organic law goes further than most: it forbids the labour and migration authorities from blocking a foreign worker's affiliation and contributions on the ground that the worker is breaching migration or labour rules. Someone who is not working joins voluntarily, on terms the Caja sets. A retiree on the pensionado route has no social-security entitlement from the visa at all; what they get instead is a statutory 15% off private hospital bills and 20% off medical consultations under the 1987 retirement benefits law.

Cover follows work
Article 77 of the organic law is universal in its terms: everyone providing services in Panama, national or foreign, employed or self-employed, must be in the Caja de Seguro Social. There is no residence qualification and no waiting period written into the article. The corollary is that nobody gets cover from a residence permit: an investor, a retiree or someone living on their own means is outside the compulsory scheme and has to join voluntarily under article 79, on terms the Caja sets by regulation rather than terms the law fixes.
The article that uncouples health cover from immigration status
Article 78 is worth reading twice. It tells the labour ministry and the migration directorate that they may not prohibit a foreign worker's affiliation to the compulsory scheme, or their payment of contributions, on the pretext that the worker is in breach of migration or labour rules. It does not legalise the worker's stay - the migration consequences remain - but it stops the two systems being used against each other, and it means a foreign worker whose permit has lapsed does not thereby lose their contributions or their cover. Nothing equivalent appears in the health or social-security law of any of the nine South American countries covered here.
What a retiree actually gets
The pensionado permit confers no health cover. What Panama offers retirees instead is a price control: Ley 6 de 1987 gives retirees and pensioners, and residents aged 55 and over if women or 60 and over if men, 15% off the total bill at private hospitals and clinics - but only where they are not covered by hospitalisation insurance - 20% off general and specialist medical consultation fees, 15% off dentistry and optometry, 10% off prescription medicines and 20% off prostheses and assistive devices. Insurers writing illness cover are required by the same article to pass the benefit through in the premium at those ages.
Country-level policy
Panamanian statutes and decrees write money in balboas, with the symbol B/. - a tourist must show B/.500, a pensioner needs B/.1,000 a month, the own-means route costs B/.300,000. None of those is a sum you would ever need to convert. Panama has no circulating banknote of its own: its Constitution forbids forced-tender paper money, and what circulates is the United States dollar, with the balboa surviving as coinage and as the unit in which the law is written. The migration regulation makes the equivalence operational without ever stating it: where an applicant's document is in a foreign currency, the regulation requires a conversion into dollars, while every threshold it sets is expressed in balboas.

Practical notes
We did not read the Caja de Seguro Social's own affiliation regulations, so this record states who may join and on what statutory footing, and says nothing about contribution rates, the minimum declared income for voluntary members or the qualifying periods for benefits.
We did not read anything from the Ministerio de Salud about access to the public hospital network by people who are not insured, which in Panama is a separate system from the Caja. The absence of a statement here is an absence of reading, not a finding.
The 15% private-hospital discount is conditioned on the person not holding hospitalisation insurance. A retiree who buys private cover trades the discount for the policy, and the law does not let them keep both.
