Access to the Public Health System

Answer

The general rule applies to you

No exception is recorded for citizens of Mexico on this topic. That is the answer, not a gap - everything below is the rule you get, unchanged.

81 exceptions recorded across 33 countries · none match Mexico to Peru · Access to healthcare

Peru's position is the most open of the four countries covered here. Since 2019 the state insurer SIS has been authorised to enrol any person resident in the national territory who holds no health insurance, regardless of their economic classification, and enrolment is free with no monthly payment. In practice the gate is documentary rather than legal: enrolment is done with a national identity document or a carné de extranjería, so it follows residency rather than arrival. EsSalud runs a separate paid scheme open to residents, nationals and foreigners alike.

Requirements

  • SIS Para Todos is open to anyone living in the country who holds no other health insurance, whatever their means
  • Enrolment carries no cost and requires no monthly payment
  • Enrolment is done with a DNI or a carné de extranjería, online, by WhatsApp, by email, through the SIS app, or in person at a SIS office
  • No socio-economic classification is required, and holders of a 'not poor' classification are eligible
  • Cover is the Plan Esencial de Aseguramiento en Salud, at Ministry of Health and regional government facilities
  • Enrolling in another insurer with PEAS cover and a complementary plan cancels SIS Para Todos cover
  • EsSalud's Seguro Potestativo is a separate paid alternative, open to any resident in the country, national or foreign, with no age limit

In detail

Four countries, four different answers

Argentina restricted ordinary care at nationally-run establishments in 2025 to citizens and permanent residents. Colombia makes enrolment compulsory for everyone resident and then sorts people into a contributory or subsidised regime by ability to pay. Uruguay lets a foreigner with no residency application at all join the public provider by paying a monthly fee. Peru offers free enrolment and free cover at public facilities to anyone resident without other insurance, means-tested for nothing. These are four different policies and a plan built on one does not transfer to another.

The gap between the rule and the card

The decree speaks of any person resident in the national territory. The enrolment page asks for a DNI or a carné de extranjería. The second document is issued after a residence status is approved, so between arriving and being registered there is a stretch where the entitlement exists on paper and the enrolment channel does not accept you. Nothing we read at SIS describes an alternative document for that window, and we do not assert one.

What applies to you

Nothing changes for a citizen of Mexico

We have no rule recorded that treats your citizenship differently here, so the general rule above is the one that applies to you. That is an answer, not a gap.

What the law says

Text layer - verbatim verifiedDecreto de Urgencia 017-2019, art. 2 · Artículo 2, numeral 2.1
«a toda persona residente en el territorio nacional que no cuente con ningún seguro de salud»

Our translation - not official

any person resident in the national territory who does not hold any health insurance

The test is residence in the territory and the absence of other cover. Nationality and immigration status are not mentioned, and neither is means: the same paragraph authorises enrolment irrespective of socio-economic classification.

«Este plan de seguro permite a todas las personas que viven en el país, y que no cuenten con un seguro de salud, afiliarse al SIS sin importar su condición socioeconómica y acceder gratuitamente a los servicios de salud en los establecimientos de salud públicos del Ministerio de Salud y gobiernos regionales. El proceso de afiliación no tiene costo para el ciudadano ni requiere un pago mensual.»

Our translation - not official

This insurance plan allows all people living in the country who do not hold health insurance to enrol with SIS regardless of their socio-economic condition and to access health services free of charge at the public health facilities of the Ministry of Health and the regional governments. The enrolment process has no cost for the citizen and requires no monthly payment.

Two separate 'free' claims: enrolment costs nothing, and the services themselves are accessed free of charge at public facilities. Uruguay's ASSE, by contrast, charges no fee to enrol but charges a monthly premium for the cover.

About this source

The public health insurer for those without other cover. Publishes who may enrol and with what documents.

Standing: Applies the rule

Cannot be cited for: Not the legal entitlement, which sits in the emergency decree and the health law - this is the operational restatement, and its document list is narrower than the decree's wording. Not clinical scope, which belongs to the health ministry. Silent on someone holding no Peruvian identity document.

«Este mensaje debe tener tu nombre, número de DNI o Carnet de Extranjería, fotos de las dos caras del mencionado documento, un número de celular para contactarte y tu dirección donde resides actualmente.»

Our translation - not official

This message must contain your name, DNI or Carnet de Extranjería number, photographs of both sides of that document, a mobile number to contact you, and the address where you currently live.

This is where the legal openness meets a practical limit. The carné de extranjería is issued once a residence status is granted, so the route is effectively open to residents rather than to anyone standing in the country on a tourist stamp.

«No requiere contar con clasificación socioecónomica (CSE). Pueden acceder a este seguro las personas cuya CSE no está vigente o tienen la CSE de No Pobre.»

Our translation - not official

It does not require holding a socio-economic classification (CSE). People whose CSE is not current, or who hold a 'Not Poor' classification, can access this insurance.

'socioecónomica' is the publisher's own spelling and is kept as printed. The point of the sentence is that this is not a poverty scheme: being classified as not poor is expressly no bar.

Practical notes

We have not sourced the amount of the EsSalud Seguro Potestativo monthly contribution, nor the contents of its plans; SIS publishes no premium because there is none.

SIS states that enrolling with another insurer that provides PEAS cover plus a complementary plan cancels SIS Para Todos cover, so holding private insurance as a visa requirement elsewhere is not a neutral act here.

The complementary plan and funeral benefit are stated by SIS as running until 31 December 2026; the underlying PEAS entitlement is not described with an end date.

Nothing here covers emergency care for people with no insurance and no document, which is governed by the Ley General de Salud - a statute we have not read.
