Peru's position is the most open of the four countries covered here. Since 2019 the state insurer SIS has been authorised to enrol any person resident in the national territory who holds no health insurance, regardless of their economic classification, and enrolment is free with no monthly payment. In practice the gate is documentary rather than legal: enrolment is done with a national identity document or a carné de extranjería, so it follows residency rather than arrival. EsSalud runs a separate paid scheme open to residents, nationals and foreigners alike.

Requirements
- SIS Para Todos is open to anyone living in the country who holds no other health insurance, whatever their means

In detail
Four countries, four different answers
Argentina restricted ordinary care at nationally-run establishments in 2025 to citizens and permanent residents. Colombia makes enrolment compulsory for everyone resident and then sorts people into a contributory or subsidised regime by ability to pay. Uruguay lets a foreigner with no residency application at all join the public provider by paying a monthly fee. Peru offers free enrolment and free cover at public facilities to anyone resident without other insurance, means-tested for nothing. These are four different policies and a plan built on one does not transfer to another.
The gap between the rule and the card
The decree speaks of any person resident in the national territory. The enrolment page asks for a DNI or a carné de extranjería. The second document is issued after a residence status is approved, so between arriving and being registered there is a stretch where the entitlement exists on paper and the enrolment channel does not accept you. Nothing we read at SIS describes an alternative document for that window, and we do not assert one.

The test is residence in the territory and the absence of other cover. Nationality and immigration status are not mentioned, and neither is means: the same paragraph authorises enrolment irrespective of socio-economic classification.

This insurance plan allows all people living in the country who do not hold health insurance to enrol with SIS regardless of their socio-economic condition and to access health services free of charge at the public health facilities of the Ministry of Health and the regional governments. The enrolment process has no cost for the citizen and requires no monthly payment.
Two separate 'free' claims: enrolment costs nothing, and the services themselves are accessed free of charge at public facilities. Uruguay's ASSE, by contrast, charges no fee to enrol but charges a monthly premium for the cover.

This is where the legal openness meets a practical limit. The carné de extranjería is issued once a residence status is granted, so the route is effectively open to residents rather than to anyone standing in the country on a tourist stamp.

'socioecónomica' is the publisher's own spelling and is kept as printed. The point of the sentence is that this is not a poverty scheme: being classified as not poor is expressly no bar.

EsSalud is the contributory social-security insurer, normally reached through an employer. This voluntary scheme is the door in for the self-employed and for anyone with no Peruvian employer.

Practical notes
We have not sourced the amount of the EsSalud Seguro Potestativo monthly contribution, nor the contents of its plans; SIS publishes no premium because there is none.
SIS states that enrolling with another insurer that provides PEAS cover plus a complementary plan cancels SIS Para Todos cover, so holding private insurance as a visa requirement elsewhere is not a neutral act here.
The complementary plan and funeral benefit are stated by SIS as running until 31 December 2026; the underlying PEAS entitlement is not described with an end date.
Nothing here covers emergency care for people with no insurance and no document, which is governed by the Ley General de Salud - a statute we have not read.
