Public hospital treatment in Grenada is charged, and a foreigner pays a third more. The Grenada General Hospital (Fees) Rules set a schedule of ward, surgery and dispensary charges and then provide, in rule 5, that patients who are not citizens of Grenada and who have been resident here for less than three months, or who are not in local employment, pay those fees increased by one-third. The Rules were made under an Act repealed in 1998 and survive because the repealing Act saved them until new ones are made; the amounts in them were last set in 1988. The National Insurance Scheme pays cash benefits and does not pay for treatment, so there is no contributory health cover to join.

In detail
A residence test inside a fees rule
Rule 5 of the Grenada General Hospital (Fees) Rules is the single most useful sentence a newcomer can read about healthcare here, and nothing publishes it. Patients who are not citizens of Grenada and who have been resident for less than three months, or who are not in local employment, pay the scheduled fees increased by one-third. The two conditions are alternatives, so there are three ways out of the surcharge: be a citizen, be employed locally, or have been here three months. A visitor taken to hospital in their first week pays the premium; the same person after three months does not. We have found no other provision in Grenadian law that turns on three months of residence.
A 1953 instrument, amended in 1988, still in force
The Fees Rules were made under the Hospitals Act and came into operation on 21 October 1953. The Hospitals Act was repealed in 1998 by the Grenada Hospitals Authority Act, and section 37(2) of that Act saved the rules made under it until new ones are made. We found no new ones. The consequence is that the law on hospital charges is a schedule of 1988 amounts - fifteen dollars a day in Duncan Ward, two hundred and fifty for major surgery, two dollars for a prescription - and a surcharge rule that is thirty-seven years older still. Section 15 lets the Authority charge prescribed fees with the Minister's approval, so what is actually charged for a service that did not exist in 1988 is not in anything we read.
The contributory scheme does not pay for treatment
The National Insurance Act insures people between sixteen and sixty in insurable employment and pays sickness, maternity, invalidity, age, survivors', funeral and employment injury benefits. Every one of those is money. There is no medical care benefit and no mechanism by which contributions reduce a hospital bill. Sickness benefit replaces sixty-five per cent of average insurable earnings during incapacity, which helps with income and not with the fee. A reader who expects a European-style social insurance system because the contribution rates look European - 13.5 per cent of pay in 2026, rising to 16 per cent by 2031 - is expecting the wrong thing.
Contributions legislated to 2031
S.R.O. 43 of 2023 did something unusual: it wrote a contribution rate into the Schedule for every year from 2024 to 2031, in quarter-point steps, taking the employer from 6.75 to 8.5 per cent and the employee from 5.75 to 7.5. It also created a new Part III for the earnings ceiling and raised it to EC$1,200 a week or EC$5,200 a month from 1 January 2024. An employer budgeting for a foreign hire can read the cost of the next six years off a single instrument, which is more than most countries in this comparison allow.
Country-level policy
Grenada publishes its law well, in two halves that do not meet. The Chapters are the Continuous Revised Edition of 2010, and most of them are amended only to the mid-2000s: the Immigration Act stops at Act 6 of 2007, the Citizenship Act at Act 31 of 2002, the Income Tax Act at Act 2 of 2010. The Acts and Statutory Rules and Orders since are published separately, by year, as enacted. Nothing in either half tells you that the other exists. Read the Immigration Chapter and you will not know that Eastern Caribbean citizens have an indefinite stay or that Schengen passports enter visa-free. Read the Income Tax Chapter and you will take the rate as thirty per cent when it is twenty-eight. Read the Banking Chapter and you will be reading an Act repealed in 2015.

Practical notes
Nothing on this record states what the public hospitals actually charge today. The amounts quoted are from the Rules saved in force, last amended in 1988. Press reporting in 2022 put newly introduced services such as computed tomography and dialysis in a range from EC$125 to EC$3,500 under the Hospitals Authority Act; we could not reach an instrument or a government page carrying those figures and publish none of them.
The Ministry of Health's website refused every capture we attempted, as did the National Insurance Scheme's. Both are cited nowhere on this record and everything here comes from statute and statutory instrument instead.
The claim that the National Insurance Scheme provides no medical care benefit rests on the National Insurance Act and the benefit, claims and payments, employment injury and self-employed persons Regulations as printed in Chapter 205, which we read for a treatment benefit and did not find one. The 2023 amending Acts and the unemployment benefit Regulations of 2023 were not read in full.
Nothing requires a visitor or a resident to hold health insurance, and nothing we read provides for emergency treatment free of charge to anyone.
The OECS Commission states that citizens of Protocol Member States have equal access to healthcare in a host state under its Contingent Rights Policy. We found no Grenadian provision giving effect to that, and rule 5 of the Fees Rules draws its line at citizenship of Grenada with no regional exception.
