What a foreigner gets is a permiso de conducir, not a licence, and it is priced by the month rather than by the year. The traffic department issues it to someone who is at least in the process of applying for temporary residence, which makes it one of the few Guatemalan procedures open before a status is granted. The file is heavy in one particular way: it needs a Guatemalan by origin to stand as guarantor, to swear that they are not already guaranteeing another foreigner, and to accept responsibility for the damage the applicant causes while driving, backed by three months of bank statements showing a minimum closing balance. A current foreign licence is translated and accepted; an expired one or none at all means a theory and practical test at an approved academy.

In detail
The guarantor is the real requirement
Everything else on the list is paperwork a determined applicant can assemble. The guarantor is not. They have to be a Guatemalan by origin, so a naturalised Guatemalan will not do; they have to swear they are not already guaranteeing another foreigner, so one willing friend cannot cover a household; and they have to accept liability for damage the applicant causes behind the wheel, evidenced by three months of bank statements with a minimum closing balance, signed and sealed by their branch. That is a materially larger ask than the guarantor the migration institute wants for a visa or a residence, which is a registered status rather than a personal undertaking about driving.
Priced by the month, which is the giveaway
A permit costs fifty quetzales a month for issue, renewal or replacement. Charging by the month only makes sense if the permit is cut to the length of the holder's migration status, which is consistent with the department accepting an application while a residence is still in process: what is being sold is permission for as long as the status lasts, not a document with a fixed term. The practical consequence is that the cost of driving legally tracks the length of the residence rather than being a one-off.
Country-level policy
A recurring condition in Guatemalan immigration practice has no equivalent in the other eleven countries covered here: for several of the routes a foreigner actually uses, a Guatemalan individual or a Guatemalan company has to register as a guarantor and take responsibility. It is needed for a consulted tourist visa, for employment-based temporary residence, for athletes, artists, researchers and academics, for permanent residence after five years as a temporary resident, and even for a foreigner's driving permit. The guarantor registers with the migration institute's Subdirección de Extranjería before the application is made, and has to prove income. Guatemala's own regulation says the detail lives in a separate rulebook on the ordinary-status register, which we have not read.

Practical notes
We did not read the Reglamento de Tránsito itself. Everything on this record is the traffic department's own statement of its requirements, including its citation of article 24 third paragraph as the authority for issuing permits to people whose residence is still in process.
There is no exchange of a foreign licence for a Guatemalan one described anywhere on the department's page for foreigners. A current foreign licence is translated and accepted as part of the permit application; it is not surrendered or converted.
Nothing on the department's page says what a tourist may do. The page is addressed to people with a residence or a residence application, and we did not find a primary statement of how long a visitor may drive on a foreign licence.
